What is a Plan Name?
A Plan Name is the official title given to a specific Medicare health insurance plan offered by a private insurance company.
Ex:

What is a Plan ID?
A Plan ID is a unique identifier assigned to Medicare health plans and prescription drug plans.
Example: “H5521-286-0”
It typically consists of three main parts:
- Contract Number: ex “H5521”
- H: Managed Care Organizations other than Regional PPO
- R: Regional preferred provider organization (PPO)
- S: Stand-alone prescription drug plan (PDP)
- Plan Benefit Package (PBP) Number: ex "286"
- Segment Number: ex "0"
